Preparing feed back form

Hi,

Here are a few questions that some of our team members have come up with that could form part of a training session feedback form. They include questions eliciting feedback on the session, the trainer and the actual presentation. Various rating parameters are used for different types of questions, some of them being:
Completely, Partially, Not at All
Excellent, Very Good, Good, Satisfactory, Poor

Understanding the scenarios in which your trainees will work after having attended the training is important. Based on this, you can provide multiple choices to your trainees to answer specific questions. The more granular and specific you make the feedback form, the more useful results will you get.

Session Feedback
1.Were the objectives of this course clear?
2.Has the course fulfilled its stated objectives?
3.List out topics that must be covered in greater depth.
4.List out topics that must be covered more briefly.
5.List out any suggestions that you may have to improve the course.
6.List out any topics that you may have noticed lacking in the course.
7.Were you encouraged to ask questions?
8.Were all your questions answered satisfactorily?
9.If your answer to question 8 was No, was a date and time communicated to you, by when your queries would be answered?
10.Did the session encourage group participation?
11.Was the group participation moderated effectively?

1.Does the course material clearly bring out the concepts described during the presentation?
2.Are the concepts brought out through sufficient examples?
3.Are the assignments: Just right, Too difficult, Too easy
4.Are the handouts relevant to the course?
